Student Information Form - The information provided on this form is used for a Choir Directory. We all use the email addresses to keep the parents informed of upcoming events.
Volunteer Sign-Up Form - This documents lists the volunteer opportunities in the choir program. If you are interested in volunteering, please fill out the form and have your student bring it to me.
Parent Letter - This is the letter that I hand out at the Parent Meeting and Open House in the Fall.
